The 27-year-old, who joined the London outfit in the summer of 2020, could see his injury-plagued spell at the club come to an end.
According to data from Fotmob, Chilwell has endured a difficult period, making just 13 appearances in the Premier League last season due to hamstring and knee problems. These persistent injuries also led to his exclusion from Gareth Southgate's UEFA Euro 2024 squad.
In another report from The Athletic, Chelsea are considering offloading Chilwell to alleviate his £200,000 weekly wage.
Chilwell is expected to leave Chelsea alongside Trevoh Chalobah, who was omitted from the club's pre-season tour squad.
