Brahim Diaz scored the visitors' opener on the stroke of half-time before Franck Kessie failed to double Milan's advantage from the spot at the hour mark.
Ante Rebic and Fikayo Tomori both found the back of the net deep in the second half to secure all three points for Milan.
The huge win sent Stefano Pioli's men to third on the Serie A standings, leaving Juve deeply frustrated.
Andreas Pirlo's side are now scheduled to travel to Sassuolo on Wednesday, May 12, during their next fixture, before hosting crowned champions Inter Milan on Saturday, May 15.
They will then wind up their disappointing campaign with a trip to mid-table to Bologna with the hope of sealing a place in the top four.
However, that will only happen if Napoli, who sit fourth lose all their three remaining matches.
Napoli are scheduled to play Udinese on Tuesday, May 11, before taking on Fiorentina away from home on Sunday, May 16.
Gennaro Gattuso's wards will then battle Verona at home in their last fixture of the campaign.
